Essential Job Functions:

  • Directs the day-to-day clinical operations of the FACT Team including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours.
  • Leads daily organizational staff meetings and treatment planning meetings.
  • Continuously evaluates the status of clients and does appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs.
  • Directs and coordinates the client admission process, treatment, rehabilitation, and support services in coordination with the psychiatrist.
  • Schedules the admission interview and develops and coordinates the initial assessment and initial treatment plan.
  • Assigns the most appropriate staff to the individual treatment team (ITT).
  • Provides clinical supervision of the development of the comprehensive assessment and the treatment plan for each client.
  • Directs and coordinates for each individual the comprehensive assessment of psychiatric history (e.g. onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
  • Participates in staff recruitment, interviewing, hiring, work assignments and orientation and performance supervision.
  • Sets up and maintains office with suitable furniture office equipment, and telephone lines directly or by delegation.
  • Develops and administers the program budget including line items to purchase necessary furniture, equipment, communications devices (e.g., telephones, cellular phones, beepers), and supplies and to cover travel and transportation (e.g. purchase or lease program cars, or cover mileage and insurance for use of personal staff vehicles or both).
  • Supervises health information management assuring maintenance of the medical record in compliance with agency policies and procedures and ensures all staff use electronic medical records appropriately and in compliance with standards from LSF, DCF,  Medicaid, and other third-party payment requirements.
  • Trains staff on the medical record requirements; regularly reviews client assessments, treatment plans and progress notes written by the staff; and supervises individual staff for medical records mastery. 
  • Carries out and documents performance improvement activities and reviews of program services.
  • Develops and maintains program policies and procedures and revises as necessary.
  • Initiates and maintains relationships in coordination with other staff, with law enforcement and other human services agencies, and with informal community resources (e.g., landlords, employers).
  • Conducts clinical assessments and provides direct clinical treatment rehabilitation, and support services to clients.
  • Performs on-call duty for crisis intervention and for staff supervision and consultation.
  • Performs related work as required.

Minimum Education and/or Experience:  Degree in nursing, social work, or other health services discipline and licensed as an RN, LCSW, LMHC, or LMFT required.  Six (6) years of strong experience providing treatment to persons with severe and persistent mental illnesses and two (2) years progressively responsible supervisory experience required. 

Knowledge/Skills/and Abilities: 

  • Extensive knowledge of mental health, psychiatry, counseling, and psychology. 
  • Extensive knowledge of methods, theories, techniques and principles of providing direct clinical treatment, consultation, and direction with children and/or adults. 
  • Extensive knowledge of the characteristics and problems of adults with severe and persistent mental illnesses.
  • Ability to supervise and guide the work of others in making complex clinical treatment decisions.
  • Knowledge of assertive community treatment and the PACT model.
  • Ability to train, advise, and assist staff in clinical/mental health decisions specific to the age and disability of the persons served. 
  • Ability to quickly assess situations and use sound judgment in defusing potentially risky situations. 
  • Ability to identify services required by persons specific to their age and disability. 
  • Ability to review client case files and treatment plans and make proper recommendations.
  • Ability to establish and maintain effective working relationships with other employees, medical and clinical professionals, and the general public.
  • Ability to handle verbally and /or physically abusive and aggressive persons and utilize aggression control techniques to ensure the safety and well-being of the person, other persons, and employees on the team.
  • Ability to incorporate cultural and age appropriate care into all aspects of communication and service delivery.
  • Ability to communicate effectively both orally and in writing with other employees, program staff, persons receiving services, outside organizations, and others.   

Necessary Special Requirements: Possession of a valid Florida driver's license, acceptable driving record, and proof of personal automobile insurance if required to drive an SMA vehicle and/or use a personal vehicle for SMA business. Complete State of Florida mandatory background screening prior to start of employment.  Complete SMA required training during the first six (6) months of employment and updated if required.

Physical: Mobility and ability to bend and reach during an 8-12 hour day. Able to lift minimum 10 pounds. Visual and auditory acuity sufficient to evaluate, intervene, treat, and record client health care needs. Fine motor skills for legible and accurate charting, daily correspondence and presentation, either manually or orally. Work endurance ability to work 8-12 hour shifts with a meal break, as possible. Routine 8-12 hour shifts. Hours and days off may vary. Extra hours may be required. Work assignment locations may vary.
